Manage Outreach Forms

Once an Outreach Form has been created, you can return to it at any time to update its content, review responses, change its status, or access its sharing options.

Outreach Forms are managed from the "Forms" area in DonorDock under the "Outreach" section. From here, you can see your existing forms, open a form to edit its settings, view the responses submitted through that form, and manage whether the form is still available for supporters to use.

View Your Outreach Forms
  1. Navigate to "Forms" in DonorDock.

  2. Open the "Outreach" section.

  3. Review the forms listed on the page.

From the Outreach Forms list, you can select a form name to open its settings or use the "View Response Report" link to the right of the form name to review submissions received through that specific form.

Outreach Forms can be in different statuses depending on where they are in their lifecycle.

Draft

The form has been created but is not yet available for supporters to use.

Published

The form is active and can be accessed through its direct link, QR code, or website embed.

Archived

The form is no longer available for new submissions, but the form and its existing response history are preserved in DonorDock.

Edit an Outreach Form
  1. Navigate to "Forms".

  2. Open the "Outreach" section.

  3. Select the name of the form you want to edit.

  4. Update the applicable settings on the left side of the form builder.

  5. Review the changes in the preview on the right.

  6. Select "Save".

You can update settings such as the form headline and message, standard Contact fields, Custom Fields, button styling, Badges, Marketing Lists, and success message.

You can continue editing published forms after they go live. Saving changes updates the published form, so you do not need to create a new form each time you want to adjust the content or fields.

The existing direct link, QR code, and embed code remain connected to the same form after normal edits.

Publish a Draft Form

New Outreach Forms begin in Draft status. A Draft can be configured and reviewed without being available publicly.

  1. Open the Draft Outreach Form.

  2. Review the form settings and preview.

  3. Save any final changes.

  4. Open the form's actions menu.

  5. Select "Publish".

Once the form is published, it becomes available for supporter submissions and the "Sharing" section appears in the form settings. The "Sharing" section provides access to the direct form link, QR code, Script Embed Code, and Iframe Embed Code.

Edit a Published Form

Publishing does not lock the Outreach Form. You can return to a published form at any time, update its settings, and select "Save" to update the live version. Because the published URL and embed remain connected to the same form, these updates do not require you to redistribute a new link or replace the existing embed code.

Before making significant changes to a form that is already receiving responses, consider how those changes may affect the consistency of your response data. For example, removing a question from a form will prevent future respondents from answering it, while earlier responses may still contain values collected before the change.

View Form Responses

You can open the response report for an Outreach Form directly from the Forms page.

  1. Navigate to "Forms".

  2. Open the "Outreach" section.

  3. Locate the form you want to review.

  4. Select "View Response Report" to the right of the form name.

You can also open the form itself and select the blue "View Responses" button at the top of the form settings page. Both options open the report containing the responses submitted through that specific Outreach Form.

Archive an Outreach Form

Archive a form when you no longer want it available for new submissions but want to preserve the form and its existing response history. Archiving is useful for forms connected to completed events, past campaigns, closed registrations, or other outreach efforts that are no longer active.

  1. Navigate to "Forms" and open the "Outreach" section.

  2. Open the form you want to archive.

  3. Open the form's actions menu.

  4. Select "Archive".

  5. Confirm the action if prompted.

Once archived, the form is removed from public use and no longer accepts new responses. Existing Form Responses remain available in DonorDock.

Delete an Outreach Form

Deleting permanently removes the Outreach Form and cannot be undone. Delete should only be used when you are certain the form is no longer needed. If the form has already been used and you want to preserve its history, archive it instead.

  1. Navigate to "Forms" and open the "Outreach" section.

  2. Open the form you want to delete.

  3. Open the form's actions menu.

  4. Select "Delete".

  5. Confirm that you want to permanently remove the form.

Because deletion cannot be reversed, review the form and any existing response history before removing it.

Keep Published Forms Current

Outreach Forms often remain in use longer than originally expected, especially newsletter signups, volunteer interest forms, and other ongoing engagement forms.

Periodically review published forms to make sure the content, Custom Fields, Marketing Lists, Badges, and instructions are still current. If a form is no longer needed, archive it so supporters do not continue submitting information through an outdated form.
