Outreach Form Examples

DonorDock Outreach Forms are designed to help your organization collect information from supporters and bring those responses directly into DonorDock. You can use them for newsletter signups, volunteer interest forms, event RSVPs, program inquiries, surveys, and other situations where you need to gather information without collecting payment.

Outreach Forms can include standard Contact fields, Contact Custom Fields, and Form Response Custom Fields. You can also configure the form to apply Badges, offer Marketing List subscriptions, and use Automations to create follow-up actions after a response is received. Each submission creates a Form Response in DonorDock while also creating or updating the associated Contact when appropriate.

Common Marketing Lists that may be offered on Outreach Forms include:

  • Newsletter

  • Volunteer Opportunities

  • Event Updates

  • Program Announcements

  • Direct Mail Marketing

The examples below show different ways Outreach Forms can collect information and support ongoing engagement with your organization.
