View and Report on Form Responses
Each time someone submits an Outreach Form, DonorDock creates a Form Response connected to that submission. Form Responses preserve the information that was collected through the form and give your organization a way to review, filter, and report on what supporters submitted.
You can review responses for a specific Outreach Form directly from the Forms page, open individual responses from the associated Contact record, or use Form Response Reports when you need to work with response data across multiple submissions.
View Responses for a Specific Outreach Form
The quickest way to review responses from one specific Outreach Form is from the Forms page.
Navigate to "Forms" in DonorDock.
Open the "Outreach" section.
Locate the Outreach Form you want to review.
Select "View Responses" to the right of the form name.
DonorDock will open a report containing the responses submitted through that form.
This report gives you a centralized view of everyone who has submitted that Outreach Form and the information associated with those responses.
For example, if you created a 2026 Gala RSVP form, selecting "View Responses" from that form lets you review submissions for that specific RSVP form without first building a separate report.
What Is Included in a Form Response?
A Form Response represents one completed Outreach Form submission.
Depending on the fields included on the form, the response may include information such as standard Contact information, Contact Custom Field values, and Form Response Custom Field answers.
Information Collected | Where It Is Stored |
|---|---|
Standard Contact Fields | Saved to the associated Contact record |
Contact Custom Fields | Saved to the associated Contact record |
Form Response Custom Fields | Saved with the individual Form Response |
Badges configured on the form | Applied to the associated Contact |
Marketing Lists selected by the supporter | Added to the associated Contact |
Review Responses from the Contact Record
Open the Contact record and review the Contact timeline to locate the Form Response activity. The response card records the form submission and lets you review the information connected to that response.
Because each submission creates its own Form Response, a Contact who completes multiple Outreach Forms can have multiple response records in their timeline.
This lets you see the history of what that supporter submitted over time, rather than only the most recent information stored on their Contact record.
View a Form Response Report
Form Response Reports can only be opened from the Forms area in DonorDock. Each report is tied to a specific Outreach Form and includes all responses submitted through that form.
There are two ways to open the response report.
From the Outreach Forms List
Navigate to "Forms" in DonorDock.
Open the "Outreach" section.
Locate the form you want to review.
Select "View Response Report" to the right of the form name.
DonorDock will open the response report for that specific form.
From the Form Settings
You can also open the response report while viewing the form itself.
Navigate to "Forms" and open the "Outreach" section.
Select the name of the form you want to review.
From the form settings page, select the blue "View Responses" button at the top of the page.
Both options open the same report and display all responses received through that Outreach Form.
What Is Included in the Response Report?
The response report shows each submission received through the form. Depending on the fields included on the Outreach Form, the report can include information such as:
The date the response was received
The Contact associated with the response
Whether the Contact was created or updated from the submission
Responses to the standard Contact fields included on the form
Responses to Contact Custom Fields
Responses to Form Response Custom Fields
Each row represents an individual form submission, allowing you to review what was submitted and how DonorDock handled the associated Contact.
Customize the Report Columns
Use the "Columns" icon on the right side of the report to control which information is displayed.
You can add columns when you need to review additional information from the form or remove columns that are not relevant to the task you are working on. This can be especially helpful for forms with a large number of fields or Custom Fields.
Column changes let you focus the report on the information your team needs without changing the Outreach Form itself.
Export Form Responses
Form Response Reports can also be exported when you need to work with the information outside DonorDock.
Use the available export options to download the report as:
CSV
Excel
PDF
Before exporting, adjust the visible columns so the report includes the information you want in the exported file.
Link an Unattached Response to a Contact
When an Outreach Form is submitted, DonorDock attempts to identify the Contact associated with the response.
In some cases, DonorDock may not be able to determine which Contact should receive the response. For example, if the email address provided on the form matches more than one Contact record, DonorDock will not automatically choose between those records.
When this happens, the Form Response is still saved, but the response report will show that it is not attached to a Contact.
You can resolve these responses directly from the report:
Open the Form Response Report.
Locate the response that is not connected to a Contact.
Select the link on that row to link the response to a Contact.
The row will expand and display options for resolving the response.
Choose whether to create a new Contact or link the response to an existing Contact.
Create a New Contact
Select "Create Contact" when the form response should be associated with a new Contact record. DonorDock will automatically create the Contact using the information provided in the Form Response and associate that response with the new Contact.
Link to an Existing Contact
Select "Link to Contact" when the response belongs to a Contact that already exists in DonorDock. A pop-up window will open where you can search for the Contact you want to use. Select the correct Contact record, then select "Save" to confirm.
The Form Response will then be associated with that Contact. This process allows your team to review ambiguous matches rather than having DonorDock automatically attach a response to the wrong Contact.
